You MUST complete all 3 pages, have notarized, and return ORIGINAL DOCUMENT to the Track.

Due to new Insurance regulations, effective JUNE 30, 2009, ALL MINORS entering the Pit Area MUST be accompanied by PARENT or LEGAL GUARDIAN with Court Order.

You MUST provide Birth Certificate for Minor and Proof of Guardianship with Valid I.D.

All Minor Release forms MUST be signed by BOTH Parents.

Minor Release Forms may be filled out during the week at the New Smyrna Speedway Track Office, or they can be faxed to you. Forms not completed at the Track Office MUST be Notarized.
